This sample script is customizable and can be used to obtain informed consent from potential respondents to the PiT survey. Informed consent is defined as the process by which someone administering a survey explains the key elements and aims of the survey, and obtains confirmation of the respondents’ willingness to participate in the survey. The script is designed to be as brief as possible, while being clear about the purposes of the survey.
